The Affordable Care Act created incentives for Accountable Care Organization (ACO) providers to coordinate patient care across inpatient and outpatient settings to avoid unnecessary duplication of services and to improve patient outcomes. The latter are often dependent on patient safety and the prevention of medical errors.
