Chronic Care Management Dollars: Who Gets Them and How?

Recognizing chronic care management is one of the critical components of primary care as of January 1, 2015, Medicare now allows qualified providers to seek payment for non-face-to-face care management/coordination under CPT Code 99490.  These services are billed separately from office visits and other face-to-face encounters, and include communication (both electronic and telephonic) with the patient and other treating health professionals for care coordination and medication management.
